(Note: To prevent the abuse, any credit card number entered will be blanked out). You can create your own forms using any HTML editor or make plug and play forms using CGI*Star professional The form IDs within the selected form will be  listed in the box. Spaces and non-alphanumeric chars in a form ID are illegal but will still be tolerated. Insert/change the characters specified below to define a form ID.  No need to define every ID and the form can have other fields  added at any time. Click 'Save Settings' to insert the form call Add the following characters in front of the Form IDs to specify * - Specify the mandatory items # - Masked items (not shown to user and in Cc email) ! - User's name field ID (e.g. User_name) @ - User's email field ID (e.g. User_email) % - Fields to be stored in file (not available in demo) Form IDs Note: The IDs within your form itself need not be changed.
